To make certain that you receive the time and attention required to provide optimal care, we ask that new patients arrive 15 - 30 minutes prior to their first appointment to complete the necessary registration process.

New patients are requested to bring their initial appointment:

1. Insurance card
2. Referral forms and co-payments
3. Copies of pertinent diagnostic tests and medical records
4. All current medications
5. Completed Patient Registration form and Physical form (if available)

To help us provide you with optimal care, if you have been hospitalized or had cardiology testing at another facility, please have copies of the reports sent to our office prior to your initial visit.
